HOW TO REFER A PATIENT

 

PRIVATE PRACTICE REFERRALS

To refer your patient to Dr Dalic, you can fax your referral to (03) 8676 4926. 

 

Alternatively, you can email your referral letter to admin@theneurogroup.com.au

 

Please ensure that your Provider Number is clearly visible on your referral, with a current contact number for your patient. 

 

All referrals will be assessed and triaged prior to making an appointment. Please provide as much detail as possible to assist in this process. Urgent referrals must be discussed with Dr Dalic (Ph: 7019 5531).
