BEFORE YOUR FIRST APPOINTMENT

Once your referral is received and triaged, Practice staff will contact you to arrange an appointment. This may either be in-person, via telehealth or telephone. You will be granted access to Dr Dalic's Patient Portal after supplying your email address. Please be sure to check your email for your Welcome email from Clinic to Cloud to complete your registration and Patient Survey.

FEES

Private fees are payable in full on the day of service. All services attract a Medicare rebate.

 

Service charges will be outlined to you by practice staff prior to your first appointment. We are able to claim the Medicare rebate for your online, therefore there is no need for you to attend a Medicare office.

 

A full range of payment options (cash, cheque, EFTPOS) are accepted on the day of your appointment. Telephone and Telehealth consultations can be paid by the Patient Portal, Direct Transfer or cheque. 

 

Bulk billing is not available.

 

APPOINTMENT REMINDERS

 

You may opt to have an appointment reminder be sent (email or SMS) prior to your
scheduled appointment. Practice staff will also attempt to remind you of your upcoming appointment, however this is a courtesy and should not be relied upon.
 

A fail-to-attend fee will be charged should you not attend your appointment and provide less than 48 hours notice. Patients with extenuating circumstances are exempt from this charge.

PATIENT INFORMATION

 

Please bring any relevant CT/MRI films/reports, blood test results and relevant past medical records to your consultation. If additional investigations are requested by Dr Dalic, please telephone/email practice staff with the date of these so that your follow-up appointment can be scheduled with her accordingly.

NDIS, VICROADS AND OTHER REPORTS

If you require NDIS, VicRoads or other reports to be completed by Dr Dalic, please notify practice staff when making your appointment. This will ensure that there is enough time allocated for this task. Please be mindful of due dates for these forms as appointment times are limited by Dr Dalic visiting Bendigo monthly only. In extenuating circumstances, Dr Dalic may complete documentation remotely which will attract a fee.
